Abstract: Long COVID is a heterogeneous post-viral syndrome with poorly defined immune mechanisms. In the LC-Optimize study, plasma proteomics (~5,400 proteins) from more than 1,000 participants across three continents identified shared signatures of persistent immune dysregulation, cellular stress, and altered hematopoietic signaling. Thirteen hallmark pathways were identified across all geographical regions and symptom clusters. These systems-level insights are now informing the LC-REVITALIZE Phase III adaptive platform trial evaluating targeted therapies for biologically defined Long COVID.
